Enrollment Procedures:

All required forms must be completed and returned to us before we will assume the responsibility of caring for your child. No Exceptions! All forms must be updated at least once per year, sometimes sooner, according to need. Please inform us immediately of any changes. Listed below you will find the list of forms that you need to return to our learning centers before your child’s first day. Most of these forms are required by the Ohio Department of Job and Family Services (ODJFS) who is responsible for supervising the state’s child care programs. When you click on a form link, you can print it directly from your screen.
